Need to contact a counselor? This form is for counseling self-referrals and individuals in need of additional support. Please complete each of the following questions in order to identify the appropriate support strategies or assistance that would be most beneficial.
All student self-referrals will be kept confidential. Administrators and the School Counselor will be the only individuals with access to the information submitted. Referrals will be responded to in a timely manner. Please allow for up to 24-48 hours for an email response.
If this is an emergency or you are experiencing a crisis outside of regular school hours please use one of the crisis hotlines below to be provided with support. In the event of a medical or mental health emergency dial 911 or go to your nearest emergency center.
